Instructions:  Parents/Guardians please complete this form if your child will be absent from school.  Please complete only one form per child.  When you click "Submit," your information will be sent to the school office automatically.  (If you need to clear the form and start over, click "Reset.")

Policy reminders:

For absences due to illness to be considered excused, parents must: 

  • Complete this form, or

  • Call the office early in the morning, 933-4750, voice mailbox 301, or

  • Send a note of explanation to the homeroom teacher on the day the student returns to school.
