Administrative Information 

Parents of students entering the 5th through 8th grades receive the EXTRA CURRICULAR ATHLETIC PERMISSION FORM and a note on sports physicals before the school year begins. In lieu of submitting a permission form before each sport's season, please ensure this form is on file with us already. If so, no additional permission forms are necessary during the school year.

 

Just a reminder: As noted on the permission form, physicals are good for 12 months only. Without a current physical on file, the student cannot participate in the extra curricular activity.  Also, please note the permission form instructions regarding required wording for sports participation.
