Administrative Information
To
participate in MHR sports in the 2011 - 2012 school year, the Diocese of
St. Petersburg requires a 4-part paperwork package. The
Participation Health Screen Form in this package REQUIRES THE PHYSICIAN'S
SIGNATURE. The four parts are listed below and can be downloaded in
pdf format at the link:

In
addition to this package, the school physical examination (Florida
Department of Health Form DH3040, School Entry Health Exam) is also
still required. All forms are valid for one year only from the date the
physician signs the "Examining Physician's Certificate" at the
bottom of the Participation Health Screen Form. No other paperwork is
required for individual sports during the school year.
